The Rheumatology rotation in Mubarak Al Kabeer Hospital is designed to provide an educational environment for the residents from different programs including general medicine, dermatology, hematology, rehabilitation medicine and family medicine. The rotation is divided into multiple services including outpatient clinics, inpatient consultations and infusion room patient care. The services are supervised by the rheumatology staff in way to allow autonomy for the residents to develop and gain experience in the field of Rheumatology.
In the outpatient clinic, residents are assigned to evaluate new referrals in a separate clinic if available then present the cases for the consultant rheumatologist with a working diagnosis and a management plan. In addition, the resident is involved in the care of the follow up cases and participate in the medical procedures in the clinic like joint injections and arthrocentesis.
For the consultation service, the resident is assigned to be the first member of the team to assess the patient and formulate a working diagnosis with a management plan. Each patient will be discussed with the attending rheumatologist in a way where the resident experience and exercise the responsibility of managing patients with rheumatic disease.
The infusion room service also provide a new experience for the residents. The resident will learn about the different biological therapies, chemotherapy and IVIG infusion protocols. The resident will also learn how to deal with infusion reactions in a secure supervised environment with trained rheumatologists and nursing staff. Furthermore, in the infusion room there is usually patients that need MSK ultrasound preformed by experienced rheumatologists. There is also a polarized microscopy in the infusion room to study synovial fluids aspirate performed by experience rheumatologists and learn how identify different types of synovial crystals.
Each Thursday we have grand round and academic activities. In grand round, all the inpatient cases will be presented by the residents to all the rheumatologists in the team. This will help to lean about cases with different complexity and how to manage them. For the academic activities on Thursdays, each week will be a different activity. The activities include: journal club, clinical examination the different joints, joint aspiration workshop with simulation, lab interpretation, MSK radiology session and problem solving case scenarios. Each academic activity is supervised by a consultant rheumatologist. Residents are also encouraged to participate in the education of other residents and the rheumatologists by providing short presentations about different topics related to the cases evaluated in the ward or OPD.
