Medical Consultations Sabah Hospital

Department of Medicine, Sabah Hospital provides medical care to the patients admitted in 16 sub-specialties hospitals around Sabah hospital in Sabah Health Region. Majority of these hospital are specialized tertiary care hospital covering the whole of the Kuwait for that particular speciality. These speciality hospitals are Maternity (Gynae & Obs), Ibn-Sina (Neurology & Neurosurgery), Al Bahar (Ophthalmology), Al-Babtain (Plastic surgery & Burns), KCCC (Kuwait cancer control centre), Zain (ENT), Sabah AL Ahmad urology centre, PMR (Physical Medicine and Rehabilitation), Razi (Orthopaedic), KCMH (Psychiatry), Addiction centre, and Chest diseases hospitals.

Generally, after the initial days of orientation we give our KB residents consultations from 2 or 3 adjacent hospitals keeping in view parking issues and weather conditions as these hospitals are at different locations within Sabah health region. Usual number of consultations given to residents varies from 5 to10 depending on situation.

The aim and salient features of this rotation includes but not limited to:
• Pre-op assessment at various speciality hospitals like Orthopaedics, Neurosurgery, Plastic surgery etc..
• Management of post-op complications at various speciality hospitals
• Management of medical issues in Psychiatry hospital esp. of long stay patients, managing and predicting the side effects of different anti-psychotic medications. As KCMH is providing mental health illness coverage to whole of the Kuwaiti population, it has almost capacity of 1000 beds (including both old and new block). Sabah medical department is providing full medical coverage to these pts 24/7. So it will give a good opportunity to our KB residents to get adequate experience of managing medical issues in psychiatric illness patients.
• Management of medical issues in cancer & immunosuppressive pts, adequately addressing the side effects of anti-neoplastic medications

Mostly consultations are given from Razi (orthopaedic), KCMH (Psychiatry), KCCC (Kuwait Cancer centre) and Ibn-Sina (Neurosurgery) hospitals.

Orientation to Rotation:

On the first day of your rotation, you should report to the site coordinator Dr. Ahmad Tabeekh (99610380) or Rotation coordinator Dr. Mohsin Mahmood (97558755) in medical department Sabah Hospital. In case of any difficulty just report to the secretary of HOD of medical department for assistance (Located at ground floor near main entrance). Then you will be given a short briefing about the orientation of rotation, daily schedule, services offered, consulting hospital and their location by the rotation coordinator. You will do on call duty every 5th day with the MTU you are attached with.

Goals at the end of Rotation
• At the end of 1 month rotation, residents should have sufficient knowledge, experience and confidence to do pre-op assessment and plan in most situations if not all, and to consult other relevant speciality professionally as deemed necessary
• Able to predict and manage post-op complications in various post-op situations
• Able to discuss the plan of management with the treating team and suggesting important changes if deemed necessary professionally
• Should have gained some experience in managing and dealing with the medical conditions of psychiatric illness patients
• Able to give medical consultation with confidence to other health care facilities

Future Plans:
• To incorporate the KB resident in evening on call rotation for consultations so that can get experience for handling acute issues in the late hours in these speciality hospitals. As currently only covering consultations in the morning hours from 8:00am to 1:00pm.
• To arrange regular MDT (Multi-Disciplinary Team) meetings with the subspecialities for complicated cases and KB resident to attend these MDT meetings with the consultant to learn about the multi-disciplinary approach in the management of the complicated cases.
