Rotation Description Form— this form is required for both new and pre-existing rotations and must be updated and resubmitted annually.

Rotation Improvement Plan (RIP) Form— this form is submitted annually with the Rotation Description form for existing rotations that want to renew their affiliation with the program. It details the improvement plans of the rotation based on their Rotation Evaluation Report.

New Rotation Application Form— this form is required for new rotations interested in joining the program.

Rotation Coordinator Job Description— this form describes the responsibilities of the Rotation Coordinator.

Fully Academic Unit Application Form— this form is required only for rotations interested in becoming Fully Academic Units (FAUs) as part of this pilot program starting 2024/2025.
